Grove St<br \/>\nMount Horeb, WI 53572<br \/>\n608-437-6902<\/strong><\/p>\n<p><em>Town of Perry representative:\u00a0 Mary Ann Nelson, 608-437-3952<\/em><\/p>\n<p><span style=\"color: #ff99cc;\"><strong><a style=\"color: #ff99cc;\" href=\"https:\/\/www.perry-wi.gov\/wp-content\/uploads\/2026\/03\/April-2026-Newsletter.pdf\" target=\"_blank\" rel=\"noopener\">April 2026 Newsletter<\/a><\/strong><\/span><\/p>\n<p>The Mount Horeb Senior Center aims to provide Case Management and Nutrition services and resources to help people 60+ remain independent and safe in their homes or apartments.<\/p>\n<p>They serve seniors in the Village of Mount Horeb and Blue Mounds and the Towns of Blue Mounds, Perry, Primrose, and Springdale.\u00a0 They are one of 15 agencies under Dane County Human Services and the Area Agency on Aging.<\/p>\n<p>Congregate and home-delivered meals are served Monday through Friday at 11:15 am.\u00a0 Reservations are needed by 1:00 pm the day before the meal.<\/p>\n<p>They offer blood pressure and foot clinics, supportive home chore services, immunization clinics, energy services, hearing evaluations, nutrition and wellness education, cards, musical entertainment, lunch outings, shopping trips, movies, darts, and a monthly newsletter.<\/p>\n<p>Transportation is provided by Care Van Services of Mount Horeb and RSVP Driver Escort service for seniors, which provides both local and trips to Madison.\u00a0 A donation is requested to RSVP of Dane County per ride received.<\/p>\n<ul>\n<li>Area Agency on Aging of Dane County: <a href=\"http:\/\/www.co.dane.wi.us\/aging\" target=\"_blank\" rel=\"noopener noreferrer\">www.co.dane.wi.us\/aging<\/a><\/li>\n<li>United Way 2-1-1: <a href=\"http:\/\/www.unitedwaydanecounty.org\/\" target=\"_blank\" rel=\"noopener noreferrer\">www.unitedwaydanecounty.org<\/a><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>SW Dane County Outreach Mount Horeb Senior Center 107 N.
