Absentee Voting

You can submit an absentee request to the clerk at MyVote.wi.gov (click on “Vote Absentee” at the top of the website).

*If Dane County voters have any problems uploading a drivers’s license, or needing a witness for their absentee ballot, or a ride to the DMV to get an ID, they can get help from the Dane County Voter ID Coaltion.  The Help Line number is 608-285-2141.

An absentee ballot can be requested at any time via mail, email, fax, or by submitting the application request form EL-121 from the Wisconsin Elections Commission.  An absentee ballot can be voted by appointment or during the in-person absentee voting hours.  Absentee ballots must be returned by the close of the polls on Election Day in order to be counted.

The request must be in writing and include:

  • Name
  • Address
  • Address to which the ballot should be mailed
  • Signature
  • Voter ID is required.  A copy of your Voter ID must be submitted with the written request.

Contact Perry Town Clerk to submit request or for an appointment.
